PRICE LIST FOR NEW-RESIDENT NAME AND ADDRESS SERVICE

- ONE TIME.. $75 computer programming and set up fee per customer.
- REFUNDABLE..$35 DEPOSIT for each Zip Code, (or set of labels), requested.
- HOMEOWNER RATES: $.35 (thirty five cents) per name supplied and billed monthly.*
- NEW RESIDENTS RATES: $.70 (seventy cents) per name supplied and billed monthly.*
*PLUS a monthly $8.50 postage and handling fee per order- not per zip code.

I have enclosed or will pay a one time $75.00 non-refundable database programming and set up fee plus a $35.00 (refundable) deposit for each Zip Code area chosen. I understand that the names and addresses of new residents for these areas will be supplied monthly on pressure sensitive labels. You will bill me monthly for these names at the rate for the subscription length I have chosen above, plus a monthly $8.50 postage and handling fee. If I have chosen the Monthly option this agreement will remain in force until I notify you to stop. Such notification must be made before the 15th of the month to be valid. I further understand that the names and invoice will be sent to me the first week of each month and that my payment is to be made no later than the 15th of the month. Furthermore, I certify that these names are for my own use only and that I will not duplicate, reuse, copy, supply, send or sell them to others.
